EXTRA SERVICE

For filling in the data you have two choices:

option 1.
You can do it yourselves. With a permanent fine liner you can fill in the data and stick a photo to the actionplan.

option2.
Or the NAN can do this for you and in that case your typed data and photograph will also be laminated.
We then need the following data of the person involved from you:

• Name;
• Date of Birth;
• diagnosed severe allergies;
• phone number of parent / care taker at:

• work,
• home
• and mobile phone;

• also we need to know who delivered the data:
• any additional instructions / data you like on the Actionplan
• and the Photo please send it as a *.jpg file.

The Costs:

The amount for (option 1) the actionplan  incl. postage paid is £ 4.00 ( 6 Euro)

The amount for (option 2)

he actionplan incl.printed delivered data, photograph and postage paid. is  £ 5.50 ( 8 Euro)
