Allergische reactie op school. Meisje, 7 jaar, Acute Astma met overgeven en algehele urticaria na het eten van snoep (M&M).
Ziekenhuisopname gedurende 6 uur.

Child, female, 7 y.o, acute asthma, vomiting, generalized urticaria, after ingestion of candies M&M, containing peanut. Reaction at school.
The emergency kit has been used: anti-H1 (mequitazine, betametasone (Célestène), Salbutamol (Ventoline). Mother called. The child is referred to the emergency unit and stays there the next 6 hours. Resolution of symptoms.
Past history: mild asthma, sensitization to house dust mites. Maternal atopy.
Specific point: A brother and a sister are allergic to peanut and to hazelnut.
Allergologic tests: prick-tests are positive to peanut: 6 mm. All legumes are negative except lupine flour: crude: 6.5 mm, cooked: 3 mm, Rast to peanut: > 100 kUI/l, DBPCFC: positive to 65 mg of peanut, with laryngeal pruritis, abdominal pain and cough in the afternoon.
Conclusions: high risk of serious acute asthma as the dominant symptom of peanut anaphylaxis. Cantine at school is contra- indicated. Care management project and emergency kit have been renewed.
Fillette 7 ans, asthme aigu avec vomissements et urticaire après consommation de bonbons M&M à l’école.
Conduite tenue : utilisation trousse d’urgence : Ventoline, Célestène, Primalan, appel téléphonique à la maman qui se déplace et conduit l’enfant au SAU. Surveillance clinique pendant 6 heures.
Antécédents : asthme modéré avec sensibilisation aux acariens, mère atopique.
Point particulier : 1 frère, 1 sœur allergiques à l’arachide et à la noisette. Consommation courante de Légumineuses sans incident.
Bilan allergologique : prick-tests positif à l’arachide 6 mm, toutes Légumineuses négatives, sauf lupin cru : 6,5 mm, cuit : 3 mm, Rast arachide > 100 kUI/l. TPO DA positif 65 mg d’arachide : prurit laryngé et douleurs abdominales, toux sèche 3 heures après.
Conclusions : anaphylaxie avec asthme dominant à faibles doses chez une enfant asthmatique. Très haut risque. Cantine interdite. PAI et trousse d’urgence. Proposition de communication du dossier au SAMU proche de l’habitation.
