Nederlands Anafylaxis Netwerk heeft het volgende bericht ontvangen:


Vrouw kreeg een systemische reactie (anafylaxis) na de consumptie van een "industriële" appeltaart . De vrouw bleek anafylactische te reageren op lupinemeel dat als "verborgen allergeen" in de taart zat.

Déclaration d’accident grave alimentaire : un cas

 

N° 05-06 (374) : BBG Saint Lô, Lundi 13 février 2006

 

  • Femme, présentant réaction systémique sérieuse après consommation d’une tarte aux pommes industrielle
  • SAU, injection IV Solumedrol® 80 mg, Polaramine® 1 ampoule et nouvelle injection polaramine® une heure plus tard. Traitement ultérieur Célestamine® pendant quelques jours. Prescription trousse d’urgence
  • Antécédents : Dermatite atopique enfance, rhinite allergique sensibilisation acariens, graminées
  • Bilan : tous prick tests aliments courants négatifs y compris farine de blé, gluten, isolat de blé et sésame. Prick test positif à deux reprises à farine de lupin extrait Allerbio (4 mm et 7 mm). Rast farine de lupin : 3 KU/L
  • Point particulier : réaction localisé de prurit péribuccal et buccal 4 mois auparavant après gâteaux secs industriels
  • Diagnostic : anaphylaxie allergène masqué lupin très vraisemblable.

  • Female, serious systemic reaction after ingestion of industrial apple tart.
  • Referred to emergency unit. Méthylprednisolone IV, dexchlorphéniramine IV. Second injection of dexchlorphéniramine one hour later. Secondary treatment: betamethasone and dexchlorphéniramine during a few days. An emergency kit, including epinephrin auto-injector is prescribed
  • Past history: atopic dermatitis in childhood. Allergic rhinitis. Sensitization to house dust mites and grass pollen
  • Prick tests to all common food including wheat flour, gluten and wheat isolate are negative. Prick test is positive to lupine flour (Allerbio extract). Specific IgE to lupine flour: 3 KU/L
  • Specific point: 4 months before, localized reaction of buccal pruritis after ingestion of biscuits
  • Diagnosis: Anaphylaxis to lupine flour (masked allergen)
